Department of Transportation (DOT) Physicals

We have DOT certified providers that are authorized to provide the examinations required by federal law to issue certain job related clearances, such as a Commercial Driver's License (CDL) medical card.

  • Eyeglasses (if vision requires a prescription)
  • Last MER card
  • Specific information about any prescription medications you are using along with dosages
  • Yearly clearance letter from Cardiologist or for other chronic conditions that may be required before clearance

  • Please let us know if your visit is considered "private" or through your company
  • A private DOT physical will have a flat fee to cover 
  • A non-private (through your company) DOT physical will not have a fee as it goes through the company

Employee Wellness

Annual reading of vitals signs done every year and employeers have certain financial incentive

Need a physical completed for school, camp or sports teams? Come stop by! You don't need an appointment and can have a physical completed the day of.

Pre-Operative Physical

Some surgeons require that a pre-operative physical must be completed before the date of your surgery. This just ensures that you are healthy and able to go under generalized anesthesia.

  • EKG (if required)
  • Urinalysis 
  • Blood work (typically required)

  • Photo ID

  • Any documentation provided by surgeon

  • Please note: in order ensure everything is processed and set for your operation date, you must have your physical completed at  least a week before your surgery
